Ao verificarmos a resposta dos marcadores de inflamação após o uso de
corticosteróides na DPOC os resultados são desanimadores. Os corticosteróides
inalados e orais não alteram significativamente as contagens de neutrófilos ou
citocinas inflamatórias na expectoração induzida86. Um trabalho recente de Culpitt e
colaboradores87 não revelou qualquer benefício dos corticosteróides inalados na
melhoria da função pulmonar, na sintomatologia ou na diminuição da percentagem de
neutrófilos e níveis de IL-8. Também não se verificaram alteraç_es do balanço
proteases-antiproteases, uma vez que os níveis de MMP-l, MMP-9, SLPI e TIMP-1
não foram modificados pela introdução de tratamento.
Os corticosteróides são ineficazes na inflamação neutrofílica, diminuem a apoptose
dos neutrófilos e não inibem os níveis elevados de IL-8 e TNF- 29,43. Existe assim
uma resistência natural da doença a estes fármacos. Causam uma diminuição do
glutatião intracelular nas células epiteliais das vias aéreas, agravando o stress
oxidativo88.
Os mediadores inflamatórios modulam o número e afinidade dos receptores dos
glucocorticóides nas células brônquicas epiteliais. A inflamação, presente nos
fumadores e doentes com DPOC, diminui a capacidade de ligação destes receptores89.
Por outro lado a SLPI é aumentada pelos corticosteróides "in vitro"90 e estudos
recentes revelam que, embora os corticosteróides diminuam a apoptose neutrofílica,
têm um papel promotor da clearance destas células dos locais inflamatórios, por
aumento da ingestão macrofágica91.
Mais trabalhos de investigação são necessários para esclarecer a relação entre a
inflamação e os corticosteróides e também para descobrir outros anti--inflamatórios

eficazes (alguns já em estudo como os inibidores da fosfodiesterase 4, a IL-10, os
análogos da PGE, ....) que actuem nas múltiplas vias, diminuindo a globalidade do
processo inflamatório.
