MATERIAIS E MTODOS
Este um estudo retrospectivo.
momento da cirurgia era de 25 anos (desvio padro de 6.1 anos), com mnima de 19
anos e mxima de 38 anos eo total do tempo de tratamento ortodntico pr-cirrgico
variou de 8 a 40 meses, com mdia de 27 meses. Este estudo foi realizado com a
aprovao do projeto de pesquisapelo Comit de tica em Pesquisada Universidade
Federal do Piau UFPI(14599813.8.0000.5214).
Foram analisados inicialmente os pronturios de todos os pacientes submetidos a
cirurgias ortognticas dos arquivos de duas clnicas particulares especializadas
nesse tipo de procedimento, localizadas em Teresina Piau - Brasil, no perodo de
Janeiro de 2008 a Janeiro de 2013. Selecionaram-se para incluso no estudo todos
os sujeitos que corresponderam aos seguintes critrios: pacientes que possuam
ngulo ANB igual ou menor que 0 e o valor de Wits negativo (menor que - 1mm
para homens e 0 mm para mulheres) sem associao com fissuras-labio-palatais ou
sndromes crnio-faciais;ausncia de assimetrias faciais graves; presena de
telerradiografia de perfil anterior ao preparo ortodntico (T0), pr-operatria imediata
(T1) e ps-cirrgicaimediata (T2) de boa qualidade quanto nitidez dos tecidos
duros e moles.
As telerradiografias T2 foram feitasantes do tratamento ortodntico ps-operatrio,
variando de 20 a 60 dias aps a cirurgia. Todos os planejamentos e cirurgias foram

Anlisedos dados
Para testar o erro intra-examinador das medidas da amostra dos 22 pacientes, dez
foram selecionados ao acaso e suas telerradiografias laterais retraadas e medidas
nos trs tempos (T0, T1 e T2), aps um perodo de 30 dias. Obtidasas mdias da

RESULTADOS
Foram comparadas as relaes entre os valores mdios e a norma e os valores
mdios entre si nos trs tempos (tabela 2).
No perodo que antecedeu ao tratamento ortodntico pr-cirrgico (T0), observou-se
que os incisivos superiores estavam compensados (U1-SN; U1-NA, U1.PP e U1.PO)
e essa inclinao foi mantida pr-cirurgicamente (T1), apesar de uma leve
descompensao nessas variavis, permanecendo os valores mdios distantes
significamente dos valores normativos para U1-SN (p<0,001); U1-NA (p<0,001) e
U1.PP (p<0,001). Aps a cirurgia (T2), a compensao dos incisivos diminuiu, mas
sua inclinao continuava distante em relao a norma para as variveis U1-SN
(p=0,001) e U1.PP (p<0,001). Em seguida as variveis foram comparadas entre os
tempos T0, T1 e T2 (tabela 2). De T0 a T2, os incisivos superiores se tornaram mais
retroinclinados, sendo essa alterao estatisticamente significativa para U1-SN (T0T2, p=0,015 e T1-T2, p=0,040) e U1-NA (T0-T2, p=0,001 e T1-T2, p=0,010). No
houve uma descompensao pr-operatria satisfatria (T0-T1).

A taxa de variao normal (porcentagem) para as variveis U1-SN e IMPA nos trs
tempos foi apresentado nas tabelas 3 e 4, respectivamente. Em T0, foi observado
que apenas 9% dos pacientes apresentavam-se com a inclinao dos incisivos
superiores dentro da faixa de normalidade, aumentando para 27% aps terapia
ortodntica.

DISCUSSO
Foi observadoque antes do incio do tratamento ortodnticopr-cirrgico (T0), a
inclinao dos incisivos superiores estava aumentada, com a mdia de U1-SN, U1NA e U1-PP (tabela 2), diferindo significativamente dos valores normativos. O
tratamento ortodntico pr-operatrio promoveu uma leve retroinclinao dos
incisivos superiores, mas sem significncia estatstica, confirmando a ausncia de
uma descompensao pr-operatria satisfatria. Essa discreta lingualizao entre
os tempos T0 e T1pode ser explicada como o resultado do fechamento de diastemas
presentes na arcada superior de alguns pacientes que so resultantes da tentativa
dentria de mascarara discrepncia esqueltica e melhorar a funo mastigatria, ou
da utilizao de elsticos classe II em alguns pacientes, pois nenhum paciente
sofreu exodontia de pr-molares e nem fizeram uso de dispositivos de ancoragem.
Diferentemente desse estudo, Troy et al6e Kim e Baek9 - no grupo sem exodontias -,
observaram um leve aumento na inclinao dos incisivos
superiores aps a
aps a terapia
ortodntica pr-operatria.J, Kim e Baek9verificaram que a ortodontia properatria proporcionou uma descompensao satisfatria nos incisivos superiores
do grupo de pacientes tratados com exodontia de pr-molares,promovendo uma

diminuio significativa na compensao desses dentes em relao as variveis U1NA e U1-SN, variveis tambm avaliadas nesse trabalho. Isso pode ser explicado
pela presena de espao na arcadamaxilar necessria para colocao adequada
dos dentes anteriores em sua base ssea como consequncia das exodontias dos
pr-molares.
A remoo de pr-molares com a finalidade de se obter espao durante o preparo
inicial pode no ser includo no plano de tratamento por diversos motivos, dentre
eles: a resistncia do paciente remoo dos elementos dentrios, o aumento no
tempo do preparo ortodntico e a formao do profissional, entretanto, vale ressaltar
que o resultado esttico e funcional obtido ao final do tratamento cirrgico superior
quando se posiciona adequadamente os incisivos nas suas bases sseas.10

mais facilmente conseguida que a dos incisivos superiores. Isso pode ser explicado
pela forma e pelo menor tamanho das razes desses dentes, bem como pela fora
da musculatura da lingua que favorece a inclinao labial dos elementos dentrios,
alm da vestibularizao como consequncia da dissoluo de apinhamentos
dentrios.
Aps a cirurgia, foi observado que os incisivos inferiores foram movimentados em
direo sua inclinaodentria anterior ao tratamento, diferindo da norma apenas
em relao varivel L1.NB.Essa diferena entre o valor normativo e a mdia pscirrgica para L1.NB, no deve ser levada fielmente em considerao uma vez que
essa varivel frequentemente influenciada pela presena de mordida aberta
anterior e alteraes verticais do tero inferior da face, caractersticas comuns em
pacientes com discrepncia esqueltica classe III,

Os nervos que do a sensibilidade para o rosto passam dentro dos ossos da face.
Portanto, pode haver diminuio da sensibilidade (dormncia) da pele na regio das
bochechas e lbio superior. Na grande maioria, essa dormncia retorna em seis
meses.

avaliao prvia com mdico anestesiologista.

TABLE FILES
Each table must be in WORD or EXCEL format and entered as a separate file. Each
table must have its own legend accompanying it, numbered with Arabic numerals and
sequentially referred to in the text. All abbreviations used in the table must be defined
in a footnote. Use * P=.05; ** P=.01; *** P=.001; ****P=.0001 as needed. Tables
cannot be in pictorial or image formats. Pictorial or image formats are figures and
must be entered as figures.
FIGURE FILES
Each figure must be of sufficient resolution for high quality publication usually in TIFF
or EPS format. All images need to be at 300 DPI when the figure is of the size to be
used in publication.
If you enter a large image at 300 DPI and reduce it to a much smaller size for
publication, this will increase the DPI and the image will be very heavy and slow to
open electronically. If you enter a small image (such as a 35 mm picture) and plan to
enlarge it for publication, it needs to be entered at more than 300 DPI since
enlargement will only reduce the resolution.
Figures in WORD or presentation software such as PowerPoint, Corel Draw or
Harvard Graphics do not contain sufficient resolution for publication and will not be
accepted. Authors will be charged for publication of figures in color.
Manuscript Review
