RESUMO
The hepatic transplant is the standard treatment for terminal hepatic illness. With the
refinement of the surgical technique, improvement of the effectiveness of the
immunosuppressive drugs, the better knowledge of the reperfusion injury and the use
of more physiological solutions of preservation, had a considerable increase of the
survival of hepatic graft. The Collins’ preservation solution used since 1969 , in renal
transplant, is simplest and most economic preservation solution. The Belzer’s
solution is used world wide and preserves hepatic graft for a longer period, although
more expensive. The present study it compares 2 projects of hepatic perfusion using
the Collins and Belzer’s solution. The difference between the two groups is that the
Belzer’s solution perfused for the portal vein is substituted by the solution of Collins.
At Walter Cantídio Hospital of the Federal University of the Ceará 49 patients
submitted to hepatic transplant of deceased donors graft were evaluated. The
hepatic function after the transplant was evaluated through parameters AST, ALT,
INR and Bilirubin in first and the seventh postoperative day. The time of cold
ischemia was less than 10 hours in all the patients. There was no difference between
the two groups in the analyzed parameters, except in the INR of the group where
Collins’ solution in the portal vein was perfused, which was greater in the 1º
postoperative day. The two projects of hepatic preservation can be used with
security, as the cold ischemia time is less than 10 hours, however, it had a reduction
of the cost of the liver transplant in the group that used a lesser volume of the
solution of Belzer.

Quando comparados o valor do INR entre os dois grupos, foi verificado que o
valor do INR no 1°dia pós-operatório foi maior no grupo 2 (p =0,04). Não houve
diferenças as concentrações séricas de bilirrubina (figura 10).

5. DISCUSSÃO
A média de idade dos receptores neste estudo foi de 52,5 anos no grupo 1 e
53 anos no grupo 2. Garcia et al (2005) mostraram idade semelhante. A grande
maioria dos doadores falecidos estavam internados no principal hospital de trauma
do estado, tinham menos de 50 anos e foram vítimas de trauma crânio-encefálico.
Uma única equipe foi responsável pela captação e pelo implante do enxerto,
garantido a homogeneidade da técnica cirúrgica entre os dois grupos.
transaminases, bilirrubinas e INR, são aquelas que melhor avaliam este grau de
disfunção (PLOEG et al. 1993).
Adam et al. (1996), compararam dois grupos num total de 550 transplantes
de fígado no Hospital de Paul Brousse de Paris. Em um grupo, utilizou apenas a
solução de Belzer e em outro grupo as soluções de Belzer e Collins, semelhante a
este estudo. Os parâmetros utilizados para mensurar a disfunção hepática foram as
transaminases no quinto dia pós-transplante, os níveis de bilirrubina no terceiro e
décimo dia pós-transplante e o tempo de protrombina no segundo e no quinto dia
pós-transplante. Foi ainda avaliado o padrão de necrose dos hepatócitos nas
biópsias rotineiras realizadas após a reperfusão do enxerto hepático. No grupo que
utilizou uma única solução (Belzer), ocorreu um aumento do tempo de protrombina
que persistiu no terceiro e no quinto dia pós-operatório, mesmo quando excluídos os
procedimentos de urgência (hepatite fulminante e retransplante), devido aos seus
riscos e complicações específicas. Ocorreu também uma tendência de aumento da
AST no grupo que utilizou exclusivamente a solução de Belzer. Ocorreu também
uma diminuição da necrose hepática histologicamente comprovada no grupo que
utilizou as duas soluções. A menor viscosidade da solução de Collins permitiu uma
melhor pressão de perfusão, contribuindo para uma distribuição mais homogênea da
solução de preservação em todo o parênquima hepático. Estes autores concluíram
que a associação da solução de Belzer e Collins na preservação de enxertos
hepáticos provenientes de doador falecido, foi fator determinante da qualidade do
órgão, contribuído inclusive na redução do sangramento intra-operatório.
Este estudo utilizou nos dois grupos o mesmo esquema de perfusão pela
aorta com solução de Collins. A diferença entre os dois grupos residia na perfusão
pela veia porta; no grupo 1, foi utilizado 1L de Belzer e no grupo 2, 1L de Collins. As
variáveis utilizadas foram às concentrações séricas de transaminases, bilirrubinas e
valores do INR no primeiro e no sétimo dia pós-transplante. O valor de INR no
primeiro dia de pós-operatório foi maior no grupo 2, que utilizou somente solução de
Collins, mas foi normalizado no 7º dia pós-operatório. As transaminases e os níveis
de bilirrubinas foram semelhantes nos dois grupos no 1º e 7º dia pós-operatório.
Foram excluídos os casos mais peculiares e complexos de transplante, como o
