RAC 05 - Içamento de Carga

Este alerta tem como objetivo ressaltar os principais pontos de atenção e riscos
envolvendo atividades de içamento de carga.
FATALIDADE – FÁBRICA NOVA – MARIANA, MG, BRASIL.

HPI N1 (31/05/2022)

Durante a movimentação de uma estrutura com

a utilização de caminhão guindauto, no interior
de uma báscula de caminhão fora de estrada,
houve perda de estabilidade e essa estrutura
tombou sobre o empregado.

durante atividade de
movimentação de carga para
troca de cilindro da báscula de
um caminhão fora de estrada.

LIÇÕES APRENDIDAS

1. Atividades de içamento devem ser isoladas e sinalizadas (requisito 5.7 a).

2. É proibido se posicionar no raio de ação da carga, em razão dos riscos de queda e
movimentação. (requisito 5.7.n).
3. A carga deve ser adequadamente amarrada, considerando seu tipo, dimensão e peso
(requisito 5.7.q).

AÇÕES IMEDIATAS

1. Identificar e garantir isolamento/sinalização de todas as atividades de içamento de

carga.
2. Garantir que os requisitos do RAC 05 sejam desdobrados nos procedimentos e
treinamentos locais.
3. Garantir que todos os executantes de atividades de içamento de carga estejam
treinados no RAC 05, procedimentos locais, operação de equipamentos de içamento e
outros treinamentos legais aplicáveis à atividade.
4. Garantir que toda atividade que envolva içamento de carga possua uma ART e, quando
aplicável, um procedimento e PTS.

During the movement of a structure with the

use of a truck mounted crane, inside the dump
of an off-road truck, there was a loss of stability

LESSON LEARNED

1. Lifting activities areas must be isolated and signaled (requirement 5.7 a).
2. It is strictly forbidden to place people under a suspended load or in its action of line,
considering the risk of falling and movement. (requirement 5.7.n).
3. The load to be lifted must be properly hitched according to its type, dimensions, weight
and the type of accessory/equipment to be used.(requirement 5.7.q).

IMMEDIATE ACTIONS

1. Identify and ensure isolation/signaling of all lifting activities.

2. Ensure that the requirements of CAR 05 are cascaded in local procedures and training.
3. Ensure that all employees involved in lifting of activities are trained in CAR 05, local
procedures, operation of lifting equipment and other legal training applicable to the
activity.
4. Ensure that every activity that involves lifting a load has an JSA and, when applicable,
a procedure and a SWP.
