A assistência domiciliar mostra-se uma alternativa viável aos cuidados hospitalares e para
redução dos gastos com pacientes crônicos estáveis, sendo necessário analisar fatores que
podem interferir na implantação do modelo, como a disposição dos cuidadores de assumir os
cuidados do paciente em casa e logística própria dos sistemas de saúde. No entanto, esta
revisão não conseguiu demonstrar que a assistência domiciliar é menos dispendiosa que os
esquemas existentes para subgrupos como pacientes que se recuperam de cirurgia eletiva ou
pacientes que necessitam de cuidados agudos.

Identificou-se redução de custos entre 23,9% e 67,1%, com variações entre os componentes
analisados e as metodologias utilizadas para o cálculo. Os benefícios incluem diminuição de

The cost-effectiveness of a nursing home-based transitional care unit for increasing the
potential for independent living in the community among hospitalized older adults
TCU patients spent, on average, 162.0 days institution-free within six months days post-
discharge, while usual care patients spent 140.6 days institution-free. TCU recipients had a
lower total cost of care, by CAN$1,106, due to the reduced hospital length of stay 15.6d for
TCU patients and 28.6d days for usual care. TCU was deemed the more cost-effective model of
care. Through rehabilitative and restorative care, TCUs can reduce hospital length of stay,
increase potential for independent living, and reduce risk for subsequent institutionalization.
