Prevalence of type 2 diabetes and impaired fasting glucose: cross-sectional study of
multiethnic adult population at the United States-Mexico border. Díaz-Apodaca BA, t
OBJETIVE: To estimate prevalence of type 2 diabetes (diabetes) and impaired fasting glucose
(IFG) in the border region between the United States of America and Mexico, by ethnic
origin and country of residence; identify risk factors associated with both conditions; and
explore the extent to which these factors account for cross-border or ethnic disparities in
prevalence.
METHODS: From April 2001 to November 2002, … a questionnaire was administered on
diabetes (self-reported) and lifestyle and a physical examination and blood sample were
obtained. A total of 4,027 adults participated in the study: 2,120 Hispanics from the Mexican
side of the border and 1,437 Hispanics and 470 non-Hispanics (of whom 385 were classified
as "white") from the U.S. side of the border.
RESULTS: The age-adjusted prevalence of self-reported … diabetes in Hispanics was 15.4%
(16.6% on the Mexican side of the border and 14.7% on the U.S. side). The age-adjusted
prevalence of IFG was similar on both sides of the border (14.1% on the Mexican side and
13.6% on the U.S. side).
CONCLUSIONS: Established risk factors for diabetes (e.g., age, obesity, and family history)
were relevant and there was an inverse relationship between diabetes and education and
socioeconomic level. While diabetes prevalence is high on both sides of the U.S.-Mexico
border, one-third of the cases remain undiagnosed, suggesting a need for development and
implementation of a public health program for prevention, diagnosis, and control of
diabetes in the region.
Performance of Glycated Hemoglobin for the Classification and Prediction of Diabetes.

Background: Although glycated hemoglobin (HbA1c) is now recommended to diagnose
diabetes, its test performance for diagnosis and prognosis is uncertain. Our objective was to
assess the test performance of HbA1c against single and repeat glucose measurements for
diagnosis of prevalent diabetes and for prediction of incident diabetes. Design and
Methods: Population-based analyses of 12,485 participants The Atherosclerosis Risk in
Communities (ARIC) Study and a subpopulation of 691 participants in the Third National
Health and Nutrition Examination Survey (NHANES III) with repeat test results.
Results: Against a single fasting glucose ≥126 mg/dl, the sensitivity (Sn) and specificity (Sp) of
HbA1c ≥6.5% for detection of prevalent diabetes were 47% and 98%, respectively (AUC
0.892). Against repeated fasting glucose (3 years apart) ≥126 mg/dl, Sn improved to 67% and
Sp remained high (97%) (AUC 0.936). Similar results were obtained in NHANES III against
repeated fasting glucose 2 weeks apart. The accuracy of HbA1c was consistent across age,
BMI, and race groups. For persons with fasting glucose ≥126 mg/dl and HbA1c ≥6.5% at
baseline, the 10-year risk of diagnosed diabetes was 88% as compared to 55% among those
persons with fasting glucose ≥126 mg/dl and HbA1c 5.7-<6.5%.
Conclusions: HbA1c performs well as a diagnostic tool when diabetes definitions that most
closely resemble those used in clinical practice are used as the gold standard. The high risk of
diabetes among initially undiagnosed persons with both elevated fasting glucose and HbA1c
suggests a dual role for fasting glucose and HbA1c for prediction of diabetes.

RESULTS— Among 3,859 total deaths were 1,846 from CVD and 1,277 from
CHD, with death rates per 10,000 person-years of 203, 97, and 67,
respectively. Multivariate-adjusted hazard ratios (HRs) for total mortality
were 2.75 (P 0.0001) for those with NF-CVD and diabetes both, 1.92 (P
0.0001) for those with NF-CVD only, and 1.49 (P 0.0001) for those with
diabetes only, relative to neither diabetes nor NF-CVD. NF-CVD was
associated with a higher hazard of death than diabetes for total (HR 1.29,
P 0.0004), CVD (HR 1.76, P 0.0001), and CHD (HR 1.88, P 0.0001)
mortality. Only the subgroup of participants on hypoglycemic agents
showed an equivalent risk of total mortality relative to participants with
NF-CVD (HR 0.93, P 0.54).
Objective: The association between low birth weight and type 2 diabetes is well established.
We studied whether preterm birth carries a similar risk. Research Design and Methods: The
Helsinki Birth Cohort includes 13345 men and women born between 1934 and 1944. Of
them, 12813 had adequate data on length of gestation, which we linked with data on special
reimbursement for diabetes medication. Results: 5.1% of the subjects had received special
reimbursement after age 40. In subjects born before 35 weeks of gestation, the odds ratio for
diabetes was 1.68 (95% confidence interval 1.06 to 2.65) as compared with those born at
term. After adjustment for birth weight relative to length of gestation, it was 1.59 (1.00 to
2.52). Conclusions: Preterm birth before 35 weeks of gestation is associated with an
increased risk of type 2 diabetes in adult life. The risk is independent of that associated with
slow fetal growth.

Population-based Case-Control Study of Diabetes and Breast Cancer Risk in Hispanic and
Non-Hispanic White Women Living in US Southwestern States . Rollison DE.
Diabetes mellitus has been associated with breast cancer, although no studies appear to
have adequately assessed the association in Hispanic women, a population with a high
prevalence of diabetes. The authors investigated this association in a population-based
case-control study of Hispanic and non-Hispanic White women living in the southwestern
United States. Breast cancer cases diagnosed in 1999–2004 were identified through state
cancer registries (1,526 non-Hispanic Whites, 798 Hispanics).
Age- and ethnicity-matched controls (1,599 non-Hispanic Whites, 924 Hispanics) were
selected from commercial mailing lists and driver's license and Social Security records.
Diabetes history was assessed through interviewer-administered questionnaires.
Having any type of diabetes was not associated with breast cancer overall (odds ratio =
0.94, 95% confidence interval: 0.78, 1.12). Type 2 diabetes was observed among 19% of
Hispanics and 9% of non-Hispanic Whites but was not associated with breast cancer in
either group. Gestational diabetes was inversely associated with breast cancer in both
ethnic groups, especially when first diagnosed at age ≤35 years (odds ratio = 0.54, 95%
confidence interval: 0.37, 0.79). In this study, diabetes was not associated with breast
cancer overall, although the inverse association with gestational diabetes warrants
further investigation.

Introduction
Statin therapy is effective for reduction of cardiovascular
events and is generally recognised as being safe and
well tolerated.
These findings have raised questions about the safety of long-term use
of statins, and led to calls for a systematic exploration of the possible
effect of statin therapy on incident diabetes. Overestimation of clinical
benefit or underestimation of risk is potentially of major public health
importance.
