ABSTRACT
Chronic venous insufficiency can be defined as the set of clinical manifestations caused by the
abnormality of the peripheral venous system. It has a poorly defined etiology, however, it is possible
to evaluate possible risk factors, which are usually multifactorial and the association between them
seems to increase the appearance and development of varicose veins of the lower limbs. This article
is a systematic review study, conducted according to the Preferred Reporting Items for Systematic
Reviews and Meta-Analyzes methodology. 246 articles were preliminarily selected for reading, and
after exclusion criteria, 42 articles were selected for the final preparation of this review. This work
covers the innumerable possible risk factors for chronic venous disease, aiming to characterize and
define the factors that really contribute to the development of this disease. Several factors
recognized in the literature are questioned in this review.

2 METODOLOGIA
Trata-se de um estudo de revisão sistemática, conduzido conforme a metodologia Preferred
Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Para identificar os artigos
acerca dos fatores de risco para doença venosa crônica, realizou-se busca por três pesquisadores
independentes nas bases Medline, no mês de outubro de 2015, com a seguinte estratégia de busca:

A busca eletrônica resultou em 1564 trabalhos. Foram acrescentados filtros limitando a
busca para estudos com humanos, disponíveis em inglês, espanhol e português nos últimos 15 anos.
Os trabalhos deveriam ser artigos, metanálises ou revisões, incluindo revisões sistemáticas e
integrativas, e os resumos completos deveriam estar disponibilizados de forma íntegra e gratuita.
Dessa forma, foram selecionados preliminarmente 246 artigos para leitura de seus resumos.
Duzentos e quarenta e seis resumos completos foram lidos na íntegra e avaliados segundo
os critérios de inclusão elencados pela equipe pesquisadora: Estudos a partir de humanos acima de
13 anos de idade; o tema principal é doença venosa/varicosa e que abrange fatores de risco. Esses
critérios resultaram na eleição para leitura de 89 trabalhos completos e exclusão de 157.

4 DISCUSSÃO
Diversos fatores etiopatogênicos têm sido aventados como possíveis causas do
desenvolvimento de varizes e doença venosa crônica. É possível, que muito desses fatores interagem
como desencadeantes ou agravantes das varizes, entretanto, alguns podem ser apenas especulações
teóricas. Os vários fatores não são excludentes entre si, e poderiam aparecer como causa única ou
coadjuvante em cada caso de varizes. Esses fatores incluem: incompetência valvular primária,
enfraquecimento da parede venosa, anastomoses arteriovenosas e microtrombose das perfurantes.
A idade foi importante fator de risco para doença venosa crônica em vários estudos. O
aumento da idade nos artigos de Rabe E et al, 2003, Robertson L et al, 2013, Khan AFA et al, 2013,
Barros Junior N et al 2010 e Criqui MH et al, 2007 está relacionado ao aumento de prevalência de
insuficiência venosa crônica. Outros estudos como de Musil D et al, 2011, Pittaluga P et al, 2008,
Chiesa R et al, 2017, e Criqui MH et al, 2007, mostram que pacientes mais idosos apresenta maior
gravidade e atingem graus de CEAP mais avançados. O artigo de Pittaluga P et al, 2008 et al
5 CONCLUSÃO
Esta revisão abrange um espectro amplo de possíveis fatores de risco para doença venosa
crônica, tendo como objetivo caracterizar e definir os fatores que realmente contribuem para o
desenvolvimento dessa doença. Nossos achados indicam que a idade, a história familiar e o número
de gestações foram os fatores de risco mais consistentes para o desenvolvimento e para a gravidade
da doença venosa. Entretanto, são fatores de risco imutáveis e, dessa forma, não são possíveis
medidas preventivas para esses fatores. No sexo feminino, outro fator de forte consistência, em
nosso trabalho, é que as varizes pélvicas estão altamente relacionadas com patologia venosa de
membros inferiores, o qual indica necessidade de investigação semiológica para as duas patologias.
