• Geral:
Fazer uma breve revisão sobre as hepatites virais e gravidez
• Específicos:
Classificar as hepatites consoante o agente viral e o seu modo de transmissão
Apresentar os efeitos que a hepatite viral exerce sobre a gravidez
Citar o quadro clínico apresentado e o método de diagnóstico utilizado
Apresentar o tipo de tratamento e as formas de prevenção

• As hepatites virais são causadas pelos:
Vírus da hepatite A
Vírus da hepatite B
Vírus da hepatite C
Vírus da hepatite D
Vírus da hepatite E

•Tipo de vírus: RNA de fita simples positiva
•Família: Picornaviridae
•Transmissão: Fecal-oral
•Replicação: Fígado
•Excreção: Bilis
•Eliminado: Fezes

Resultados clínicos do VHA na gravidez
• Não foi relatada teratogenicidade associada ao VHA;
• Não existe casos relatados de transmissão vertical.

Resultados clínicos do VHB na gravidez
• A infecção aguda por VHB durante a gravidez é geralmente benigna;
• Não está associada a risco aumentado de morte ou teratogenicidade.

Tratamento
• Os principais objectivos do tratamento da infecção VHB crônica na
gravidez são manter a função hepática estável na mãe e prevenir a
infecção neonatal:
Tenofovir - 300mg/d/40s
Telbuvidina - 600mg/d/40s
(Brown et al., 2016; Castillo et al., 2017; Pawlowska et al., 2016; Rogan & Beigi, 2019).

Prevenção
• A infecção por HCV não influencia a via de parto e não é uma contra-
indicação à amamentação

Epidemiologia
• Mulheres grávidas que viajam para áreas endémicas, têm alto risco de
adquirir essa infecção, desenvolver hepatite fulminante e até morrer.

Resultados clínicos do VHE na gravidez
• A hepatite E fulminante ocorre com mais frequência durante a gravidez.
• Até 20% a 25% das mulheres grávidas podem morrer no terceiro trimestre.

Anamnese e exame físico: Fadiga, mal-estar, febre, dor muscular, enjoo,
vómitos, dor abdominal, constipação ou diarreia.

CONSIDERAÇÕES FINAIS
• O tratamento das hepatites virais e gravidez requer a avaliação do risco de
transmissão para o neonato, a determinação da idade gestacional no
momento da infecção e o risco de descompensação da mãe e a compreensão
dos efeitos colaterais aos medicamentos antivirais.
