Caso Clínico
Homem de 68 anos busca o pronto-atendimento por tosse há 5 dias. Refere que no início
do quadro sentia dispneia aos grandes esforços e tosse com secreção, ocasionalmente
com laivos de sangue. Hoje sua dispneia se intensificou e pela manhã a tosse produziu
pequena quantidade de sangue vivo, fato que a motivou a buscar o pronto atendimento.
Nega febre, calafrios ou suores noturnos. De nota, o paciente possui lúpus eritematoso
sistêmico (LES) e insuficiência cardíaca.

O paciente foi tabagista por 20 anos, fumando um maço por dia, cessando quando teve o
diagnóstico de LES. Nega outras drogas. Possui ainda hipertensão arterial sistêmica e
doença do refluxo gastroesofágico. Atualmente em uso de: enalapril, furosemida
160mg/dia, ácido acetilsalicílico, atorvastatina, hidroxicloroquina, prednisona 10mg/dia,
colecalciferol e omeprazol.
O LES parece bem controlado há bastante tempo, porém vale a pena indagar
sobre sinais e sintomas que fortaleçam a possibilidade de atividade lúpica e
atentar para indícios no exame físico (ex.: úlcera orais, alopecia) e exames
complementares (ex.: plaquetopenia, achados eletrocardiográficos sugestivos
de pericardite).

Demais exames com: creatinina 1,2 mg/dL (creatinina basal da internação passada: 1,0
mg/dL), ureia 45 mg/dL, sódio 129 mmol/L, potássio 4,7 mmol/L, bilirrubinas totais 0,5
mg/dL (VR < 1,2 mg/dL), aspartato aminotransferase (AST) 62 U/L (VR < 40), alanina
aminotransferase 70 U/L (VR < 55), proteína C reativa 164 mg/L, glicemia 102 mg/dL.
