The agenesis of maxillary lateral incisors: closing or reopening the
space?

Resumo
A maioria dos ortodontistas já tratou ou tratará em sua rotina ortodôntica, pelo menos
um paciente com agenesia de um ou ambos os incisivos laterais superiores, ou com
alguma discrepância de ta-manho dentário. Em busca dos objetivos ortodônticos de
estética dental e facial, função e saúde do sistema estomatognático e estabilidade dos
resultados atingidos, todos os elementos de diagnóstico devem ser clara e
minuciosamente analisados e ponderados para a elaboração de um planejamento
ortodôntico individualizado. É impossível atingir esses objetivos se o profissional eliminar
ou omitir o diagnóstico diferencial científico que pode levar ou não à extração de dentes
ou mal administrar a agenesia de um incisivo lateral superior aliada a falta de habilidade
artística no manuseio de seu instrumento de trabalho. O artigo está ilustrado com dois
casos clínicos finalizados apresentando agenesia de incisivos laterais superiores,
contribuindo para a compreensão no diagnóstico, plane-jamento e tratamento de casos
semelhantes.

CASO CLÍNICO 1
TRATAMENTO ORTODÔNTICO
O tratamento ortodôntico foi realizado a longo prazo (5 anos). A finalização foi obti-da
pelo inter-relacionamento com a prótese sobre implante nos incisivos laterais. Os va-
lores cefalométricos demonstram ter havido um significativo crescimento
mandibular do tipo C23, o que dificultou sobremaneira a mecânica
ortodôntica, mesmo optando-se pela manutenção do perímetro da
arcada dentária por meio da reabilitação utilizan-do-se implantes nos
locais das agenesias dos incisivos laterais.

Pode-se dizer que na busca desta ex-celência não existem técnicas valores
milagrosas. Existem, sim, planejamentos bem ou mal realizados e casos cefalométricos
bem ou mal finalizados. Materiais revolucionários já estão dispo-níveis inicial e final,
no mercado, porém, sozinhos, não fazem milagres. O profissional deve baseados nas
conhecer e dominar a técnica, embasados no diagnóstico correto. medidas

plexos em busca dos melhores resultados estéticos e funcionais possíveis, alicerçados no
correto diagnóstico, em todos os casos de maloclusões.
ABSTRACT
Orthodontists struggle daily with the absence of one or both maxillary lateral incisors.
This absence introduces an im-balance in a potential esthetic region of the mouth and
present a problem which complicate both orthodontic treatment and prosthetic
procedures. Orthodontics and Cosmetic Dentistry must decide between space opening or
subsequent prosthetics lateral incisor replacement and contouring the canines to
resemble lateral incisor and positioning them to function in place of the missing lateral
incisors. Such decision will be influenced by the position, size and canine morphology.
The article describes the treatment of a case in which canines are used to replace
congenitally absent lateral incisors. In another, space was opened for implant-prosthetic
purposes. In both cases consistent esthetic and functional results was achieved.
