Discussing Complementary Therapies with Cancer Patients: What should we be talking about?
01 The use of complementary and alternative medicine (CAM) has become the norm for many
02 cancer patients. Large surveys published in this issue of the Journal of Clinical Oncology demostrate
03 that at least two thirds of all cancer patients are engaged in some form of complementary or
04 alternative health practices.
05 Richardson et al at the M.D. Cancer Center, conducted cross-sectional prevalence surveys on
06 the use of CAM among patients in their clinics. On average, patients using CAM were found to use
07 four to five different therapies. Expectations for CAM were quite high: patients expected CAM to
08 improve their quality of life, alleviate symptoms, prolong life, cure their disease and boost their
09 immune system. Patients with incurable disease particulary valued CAM for its medicinal potential.
10 Among the reasons cited for use of CAM, patients included hopefulness, lack of toxicity, and a desire
11 to have more control over their medical care.
12 The related study issued by Boon et al was a population-based cross-sectional prevalence survey
13 of use of CAM among women in Ontario, Canada, who had previously been diagnosed with breast
14 cancer. The prevalence rate for CAM practices was 67% and again patients identified their desire to
15 boost the immune system, enhance their quality of life, gain control over their life, prevent or treat
16 their cancer, and side effects of cancer therapy as the principal reason for using CAM.
17 Unfortunately neither set of surveys captured extensive data on conventional medical
18 treatments for conducted formal evaluations of health-related quality of life among study
19 participants.
20 CAM use is affected by a variety of demographic, social, clinical and person factors. Education,
21 wealth, sex, and age all influence prevalence rate for CAM utilisation. Over the past decade, there has
22 been a steady increase in the prevalence of CAM among cancer patients. The reasons for this increase
23 include public interest in natural and holistic therapies, the creation of a market place for CAM
24 products and practitioners, the easing of labelling regulation on dietary supplements and the
25 dissemination of CAM information through mass media and the internet. In addition, the definition of
26 CAM has been broadened to include dietary and lifestyle behaviours as well as personal philosophies
27 or religious sentiments that were nor formerly considered part of unorthodox or alternative medicine,
28 back when those phrases implied the exclusive use of unproven medicinal therapeutics such as
29 laetrile.
30 The great majority of patients using CAM probably do so in conjunction with standard cancer
31 therapies, and not to the exclusion of oncology treatments that physicians endorse. A concern is that
32 these adjunctive practices might pose risks to patients. There are theoretical reasons for thinking that
33 some vitamins or herbal supplements might biochemically interfere with the chemotherapy or
34 radiation treatments, adversely alter treatment compliance, or cause side effects such as hepatic
35 injury.
36 There is little evidence that CAM practices work in a medical sense, so why are cancer patients
37 using CAM? The answers are very honest and compelling: whereas some patients may believe they
38 are directly treating their malignancy, most cancer patients try CAM to make themselves feel better,
39 to give themselves more control of their health and to sustain hope.
40 The suggestion that CAM is often not about cancer treatment but about feeling better and
41 about having greater control over one’s destiny argues that the most important thing to do about
42 CAM is to talk about it. Oncologists need “to ask the unasked questions”. We need to understand
43 what patients are expecting and seeking from CAM and how they chose their particular practices. This

44 requires sensitivity to the cultural, social and ethnic diversity among our patients.
45 Discussion originating from patient use of CAM represent an opportunity to forge a strong
46 relationship with our patients by listening carefully to their concerns about health and illness in the
47 broadest context.
48 In sum, the interest in CAM is an understandable expression of the hopes, concerns and
49 symptoms experienced by cancer patients. Such interest poses a challenge to oncologists: a challenge
50 not to our scientific credentials or clinical intentions, but a challenge to be better doctors – to treat
51 the disease and the patient.
