As leses meniscais
do joelho

O
s meniscos do joelho foram descritos inicialmente como
estruturas vestigiais sem funo e desta maneira foram
rotineiramente removidos durante cirurgias realizadas entre
1800 e o incio do sculo XX.
No ano de 1885, Annandale afirmou que a exciso da cartilagem
semilunar resultava na perfeita restaurao do movimento articular,
o que confirmava a viso da poca quanto pequena importncia
dos meniscos na mecnica articular.
As resseces totais dos meniscos (meniscectomias totais) rea-
lizadas por via aberta continuaram inabalveis at o ano de 1936,
quando King demonstrou as alteraes degenerativas do joelho,
que surgiram aps as cirurgias, apontando para a necessidade de

Controvrsias existem
a respeito das leses ra-
diais (fig. 10). As leses
radiais parciais preser-
vam as fibras longitudinais
circunferenciais, o que
preserva a funo meniscal de amortecimento de cargas, porm a
localizao se estende frequentemente para a zona avascular, o que
limita a chance de cicatrizao. Particularmente nas leses radiais
pequenas, o desbridamento dos bordos livres geralmente suficiente.
As leses radiais completas, entretanto, atingem as fibras longitu-
dinais circunferenciais, o que compromete a propriedade de amorte-
cimento de carga, o que significa o mesmo que uma meniscectomia
total. Tais leses quando deixadas sem tratamento, o carregamento
sobre o menisco poder provocar a extruso do mesmo para fora do
espao articular, o que acarreta perda de funo do remanescente
meniscal.
A influncia do tamanho das leses nos ndices de falha dos
reparos meniscais no completamente conhecida. Alguns autores
consideram que os ndices de falha so maiores nas leses mais
extensas (> 4 cm).
O tecido meniscal de um paciente com idade superior a 40 anos de
idade apresenta uma celularidade menor e uma resposta de cicatriza-
o menor quando comparado ao paciente mais jovem. As recidivas
de leses so mais frequentemente encontradas nos pacientes com
idade superior a 30 anos. Os dados da literatura atual sugerem que

aneurisma de vasos popl-
teos so descritos nos tratamentos meniscais artroscpicos, quase
exclusivamente nas resseces dos cornos posteriores e no nos
reparos meniscais.
A artria genicular lateral encontra-se em risco para ser perfura-
da por uma agulha nas tcnicas de sutura de dentro para fora ou
