The C2-C3, and to a lesser extenent, the C3-C4 interspace in children, have a normal physiologic displacement. In a study of
161 children (35), marked anterior displacement of C2 on C3 was observed in 9% of children between 1 and 7 years old. In a

Bicepslabral instability is evidenced by labral displacement of 5mm or more with traction on the biceps tendon, hemorrhage,

Neurologic injury is common (13% to 65%) because the glenohumeral joint is in close proximity to the brachial plexus.64 An
axillary nerve injury is the most common (73%), but often it is not clinically relevant. In a number of studies, up to one third
of patients have EMG evidence of injury, but only 5% having clinically detectable or relevant symptoms. 32,302,340 This injury
occurs because of traction as well as direct pressure on the nerve as it travels inferior to the subscapularis and below the
capsule of the glenohumeral joint. Patients with isolated nerve injuries tend to be younger and male, whereas those with
multiple-nerve lesions are more likely to be older than 60 and female.302 Unfortunately, the ability to recover from neurologic
injury decreases with age leaving some older patients quite debilitated.
Vascular injuries are rare and are typically seen in older patients who have more fragile vessels. Injury to the axillary artery
and vein are the most common injuries characteristically effecting the second part of the vessels, directly behind the
pectoralis major.6 The artery is relatively fixed at the lateral margin of pectoralis minor and becomes taut with abduction and
external rotation which makes it prone to injury with dislocation or reduction. Occlusion of the artery and vein is more common
with luxatio erecta (inferior dislocation).166 Injury with reduction is seen with the elderly, specifically in chronic dislocations
with attempted reduction.91 Signs and symptoms include a dysvascular arm or expanding hematoma. Ligation has poor
outcomes.177 Urgent consultation with a vascular surgeon is mandatory.

Galeazzi fractures consist of a fracture of the radial shaft with dislocation of the distal radioulnar joint. They are subclassified
according to the distance of the radial fracture from the articular surface. Type 1 fractures occur within 7.5 cm of the articular
surface of the distal radius and type 2 more proximally. The relevance of this classification lies in that type 1 are associated
with a significantly higher rate of instability of the DRUJ, frequently requiring open repair of this joint.146 DRUJ dislocations
associated with Galeazzi fractures can additionally be classified as simple or complex. Simple dislocations readily reduce after
radial alignment has been restored, whereas complex dislocations are those in which the DRUJ is irreducible after anatomic
reduction of the radial shaft fracture.23 Interposition of the extensor carpi ulnaris (ECU) or extensor digiti minimi (EDM)
between the distal radius and ulna have been described as causes for DRUJ irreducibility.

Late Diagnosis. If the diagnosis of DDH is not made early, preferably in the newborn nursery, secondary adaptive changes
develop (47). The most reliable physical finding in late-diagnosed DDH is limitation of abduction (Fig. 23-10). Limited
abduction is a clinical manifestation of the various degrees of shortening of the adductor longus that are associated with hlp
subuxation or dislocation (49). Other manifestations of late-diagnosed DDH may include apparent femoral shortening, also
called the Galeazzi sign (Fig. 23-11); asymmetry of the gluteal (115), thigh, or labial fulds (116); and limb-length inequality (Fig.
23-12). In patients with bilateral dislocations, clinical findings include a waddling gait and hyperlordosis of the lumbar spine
(Fig. 23-13A).
If DDH goes undetected, normal hip joint growth and development are impaired. With increasing age at detection and
reduction, and particularly in children older than 6 months, the obstacles (intraarticular and extraanicular) to concentric
reduction become increasingly difficult to overcome by simple treatment methods such as use of the Pavlik harness, and closed
or open reduction usually must be performed under general anesthesia. Restoration of normal acetabular development is less
likely as age at detection increases.

Static and dynamic forces tend to displace the patella laterally. Brattström first described the Q angle as an angle formed by
the line of pull of the quadriceps mechanism and that of the patellar tendon as they intersect at the center of the patella.
Clinically, this angle is represented by the intersection of a line drawn from the anterior superior iliac spine to the center of the
patella with a second line drawn from the center of the tibial tuberosity to the center of the patella (Fig. 47-1). For this
measurement to be accurate, the patella must be centered on the trochlea by flexing the knee 30 degrees. In males, the Q
angle normally should be 8 to 10 degrees; in females, the normal angle is 15 degrees ± 5 degrees. This valgus angle gives a
lateral force vector to the patellofemoral joint as the knee is extended. The factors that can increase this Q angle are genu
valgum, increased femoral anteversion, external tibial torsion, a laterally positioned tibial tuberosity, and a tight lateral
retinaculum. The Q angle also can be increased in a dynamic mode by internally rotating the femur.

O’Driscoll Type I coronoid tip fractures are typically associated with fractures of the radial head and a concomitant elbow
dislocation, the “terrible triad” injury of the elbow. These injuries typically occur with a PLRI mechanism, shearing off the
anterolateral radial head and coronoid tip while dislocating. Type II anteromedial coronoid fractures are seen with
posteromedial rotatory instability (PMRI) and are almost always have a concomitant avulsion of the LCL. Type III basal
coronoid fractures are most commonly associated with fractures of the olecranon and proximal ulna and have a more direct
posterior injury mechanism. They typically have larger fractures of the radial head and less ligamentous injuries. The
management of coronoid fractures is best understood by considering the patterns of injury rather than focusing on the isolated
treatment of the coronoid.

Notably, tranexamic acid (TXA) is a synthetic derivative of the amino acid lysine that functions as an antifibrinolytic agent and
