Introdução
O uso frequente e regular de medicamentos para o tratamento agudo de
episódios de cefaleia, em pacientes com cefaleias primárias pré-existentes, pode
resultar em um aumento progressivo da frequência de crises e transformação
em dor diária ou quase diária.1-3 Esta condição é secundária e denominada de
cefaleia por uso excessivo de medicação (CEM), descrita no grupo 8 da
Classificação Internacional de Cefaleias, 3ª edição (ICHD-3).4
CEM-T (de triptanos): observados 15.7% (n=104) dos pacientes de Viana e cols.8
Os pacientes deste grupo apresentam menor número de dias com cefaleia
intensa e de dias de uso de medicação por mês.2,5
A CEM pode ser distinguida como simples (Tipo I) ou complexa (Tipo II).
Casos simples envolvem uso excessivo de drogas em um período relativamente
curto, quantidades relativamente modestas de medicamentos em excesso,
contribuição psiquiátrica mínima e nenhum histórico de recaída após a retirada
da droga. Em contraste, casos complexos geralmente apresentam múltiplas
comorbidades psiquiátricas, em especial transtornos de personalidade, sendo
transtornos de personalidade borderline e histriônico os tipos mais encontrados
entre os pacientes com CEM refratária e com histórico de várias recaídas.8,9
