● Primoinfeção: varicela-zoster;
● Reativação da infeção latente surge a zona

Transmissão
● Vias respiratória e de contacto
● Contagiosa nos 2 dias antes do exantema e até que desapareçam as crostas

Doença de evicção escolar obrigatória

(a) Aciclovir oral: pode ser considerado na criança com fonte de contágio intrafamiliar, iniciando nas primeiras 24h de exantema; (b) No imunodeprimido, após 48h
de terapêutica endovenosa, pode passar a via oral (ou valaciclovir) se: melhoria clínica; apirexia e sem novas lesões há 24h; sem envolvimento de órgão; boa
tolerância oral. Iniciar desde que haja evidência de doença ativa; (c) Varicela em RN de mãe com varicela que se manifestou 5 dias antes e até 2 dias depois do
parto ou na presença de complicações graves de varicela; (d) Assegurar boa hidratação para evitar a sua precipitação nos túbulos renais. A academia Americana de
Pediatria recomenda 10 mg/kg/dose 8/8h se < 2anos.

Indicações: (amp. 125U/5mL, 500U/20mL)

• Imunodeprimidos e grávidas suscetíveis; 1 mL/kg (25U/kg)

• RN de mãe com varicela que se manifestou entre 7 dias endovenoso
1 mL/min
antes e até 7 dias pós-parto; toma única
• RN prematuro, hospitalizado, > 28 semanas de idade
gestacional, cuja mãe não tem antecedentes de varicela
ou é seronegativa; Usar até 10 dias após contacto.
• RN < 28 semanas ou ≤ 1000 g de peso ao nascer,
O efeito protetor persiste durante 3 semanas.

Caso o doente tenha indicação para tomar a vacina, esta deve ser adiada

• Mulheres não imunes antes da gravidez;
• Pais de criança jovem, não imunizados;
• Adultos ou crianças que contactam habitualmente com doentes imunodeprimidos,
 Varicela Profilaxia

Isolamento Quimioprofilaxia

Respiratório e de contacto em doentes internados (8-21
dias após contacto e até cicatrização das lesões se
infetado).
Mãe com início de doença 5 dias antes e até 2 dias após
parto deve ser isolada do RN.
Casos com indicação para profilaxia em que não é possível
administrar gamaglobulina.
Aciclovir – 20 mg/kg/toma (máx. 800 mg/toma)
4 tomas/dia, 7 dias
com início 7 a 10 dias após exposição.
 Varicela Caso clínico
Uma criança previamente saudável de 4 anos, sexo feminino, sem antecedentes pessoais ou familiares de relevo, recorre ao SU
de Faro por erupções cutâneas vesiculares generalizadas há 4 dias associadas a febre (Temp. máxima de 39ºC) de difícil
cedência ao paracetamol, prurido generalizado e tumefação, calor e dor no joelho direito com dificuldade na mobilização.

Observação na admissão: MCDTs realizados à admissão:

- Lesões nodulares, vesiculosas, pustulosas,
algumas crostosas com afetação da face, couro
cabeludo, pescoço, tronco e extremidades, com
afetação palmo-plantar. Orofaringe com lesões
nodulares na mucosa do palato mole. Joelho
direito apresenta edema, muito doloroso à
palpação, com limitação da mobilização e
impotência funcional para a marcha, com
aumento de temperatura local do joelho.
Apresenta uma lesão a nível da rótula.
- Ecografia joelho direito: "Não se detetou derrame
articular. Espessamento e hiperecogenicidade difusa do
tecido celular subcutâneo na região genicular anterior.
Bursite pré-patelar."
- Análises: a destacar Leuc 14.2x10^9/L, Neut
11.1x10^9/L, PCR 145 mg/L.
- Exame sumário de urina: urina aspeto límpido, com
proteínas, nitritos e leucócitos negativos.
- Urocultura: negativa.

 
 Varicela Caso clínico
Hipóteses diagnósticas: Varicela e Celulite do joelho direito.
Optou-se pelo internamento em Pediatria para vigilância clínica, medidas sintomáticas e tratamento com Aciclovir
1500mg/m2/dia 8/8h e Flucloxacilina 190mg/Kg/dia 6/6h + Clindamicina 30mg/Kg/dia 8/8h.
 
» Ao longo do internamento, manteve-se hemodinamicamente estável, tendo cumprido 8 dias de Aciclovir 1500mg/mg/dia, 9 dias de
Clindamicina IV 30mg/Kg/dia, 13 dias de Flucloxacilina IV 190mg/Kg/dia e 1 dia de Flucloxacilina oral 200mg/kg/dia.
» Apresentou evolução clínica favorável, mantendo-se apirética a partir de D4 e apresentando melhoria progressiva das lesões da
varicela. Apresentou também melhoria progressiva dos sinais inflamatórios ao nível do joelho direito, assim como melhoria analítica
com descida dos parâmetros inflamatórios.
 Varicela Caso clínico
MCDTs no internamento:
- HC: negativa.
- Ecografia joelho direito (05/05): "Não se observou derrame articular no recesso supratelar. Marcado espessamento,
hiperecogenicidade e edema do tecido celular subcutâneo, na face anterior do joelho, assim como a bursite pré patelar
previamente identificada."
- Ecografia joelho direito (12/05): Observa-se redução do edema do tecido celular subcutâneo pré-patelar e da região da coxa
comparativamente a estudo prévio. Bursite pré-patelar em melhoria. Não se observou derrame articular.
- Análises: (06/05 e 11/05): trombocitose, em provável contexto reativo, sem alterações da função renal, transaminases e
ionograma. Descida franca da PCR.
- RM do joelho direito.
