 Considerações iniciais
Final da década de 70 - Por intermédio da Divisão Nacional de Pneumologia Sanitária do
Ministério da Saúde, iniciou as primeiras gestões junto às instituições governamentais e
sociedades médicas no sentido de definir as medidas necessárias para controle e assistência
das Infecções Respiratórias Agudas (IRA).

1981 - IRA representava a primeira ou segunda causa de morte entre os menores de 5 anos, em
particular, entre os menores de 1 ano.

1984 - Ministério da Saúde publicou a primeira versão das normas de assistência e controle das
IRA, já incorporadas ao conjunto de ações que compunham o Programa de Atenção à Saúde da
Criança.
 Considerações iniciais

Assim foram selecionados indicadores epidemiológicos e estabelecidos parâmetros

operacionais a partir de estudos de demanda desenvolvidos no Brasil, de programas
similares de outros países e de experiências localizadas.

Na tentativa de captar e tratar o maior número de casos, estabelecem- se um grupo de

sinais clínico que, por sua capacidade de predição, possibilitassem o diagnóstico por graus
de gravidade crescentes em leves,moderados e graves. Portanto não utilizava a
denominação usual das doenças

1987- As normas foram revisadas e editadas, ainda utilizando metodologia baseada na

classificação prognóstico de casos. Entretanto, a avaliação feita em 1988 em capitais
brasileiras, com o objetivo de medir a sua aplicabilidade e aceitabilidade, pois o resultado era
insatisfatório, com aumento do uso de antibiótico excessivo de casos clasissificdos
erroneamente como moderados.
 Considerações iniciais

1986 - A Conferência Internacional sobre Promoção da Saúde estabeleceu uma série de

princípios éticos e políticos, definindo os campos de ação para a promoção de saúde para a
melhoria da qualidade de vida e saúde, incluindo a participação do controle desse processo.

A lei 8.080/90, nos seus artigos 3 e 4, diz que a saúde tem como fatores determinantes e
condicionantes a alimentação, moradia, o saneamento básico, o meio ambiente, a renda, a
educação, o lazer e o acesso aos bens de serviços essenciais.

1991 - A estratégia Saúde da Família começou a ser implantada, com a criação do Programa
Agentes Comunitários de Saúde (ACS).

 AIDPI - Atenção Integrada às Doenças
Prevalentes na Infância

Uma metodologia utilizada para reduzir a

mortalidade infantil no Brasil por meio do auxílio
a profissionais de saúde que atendem a
crianças de até 5 anos de idade em
estabelecimentos de atenção primária, como
consultório, unidade básica de saúde ou serviço
ambulatorial dos hospitais.

crescimento, desenvolvimento
físico e mental, nutricional, dentre
outros, compreendendo a criança
e o adolescente como um ser em
desenvolvimento com suas
particularidades.
PNIAM - Programa Nacional de Incentivo ao
Aleitamento Materno
O aleitamento materno é a estratégia
que isoladamente mais previne
mortes em crianças menores de
cinco anos, visto que o leite materno
é superior a qualquer outro leite
nessa fase da vida, pois é um
alimento completo que possui todos
os nutrientes que o bebê precisa,
sendo de mais fácil digestão.
Além de alimentar o bebê, o leite
materno possui anticorpos que o
protegem contra diversas doenças,
como diarreia, infecções
respiratórias e alergias.
PNI - Programa Nacional de Imunização

O PNI é responsável por definir a

política de vacinação do País,
desde a aquisição dos
imunobiológicos até a sua
disponibilização nas salas de
vacinação, estabelecimento de
normas e diretrizes sobre as
indicações e recomendações da

infecções das vias aéreas
superiores, que
compreendem rinofaringite,
faringite e amigdalite, otite
média, sinusite e laringite; e

inferiores, tais como
bronquite, bronquiolite e as
pneumonias.
Pneumonia
Causas
A pneumonia ocorre como resultado da interação entre o agente infeccioso, o hospedeiro e
problemas ambientais.

Os fatores de risco para o desenvolvimento de pneumonia em crianças são baixo peso no

nascimento, desnutrição, falta de aleitamento materno, deficiência de vitamina A, poluição
ambiental e exposição à fumaça de cigarro. A desnutrição e baixo peso no nascimento
constituem também fatores de alto risco de óbito por pneumonia aguda.
Tratamento
Antibióticos de primeira escolha são a amoxicilina na dose de 50 mg/kg, dividida em
duas tomadas a cada 12 horas por três a cinco dias após o desaparecimento dos
sintomas clínicos. A resposta clínica com desaparecimento da febre e da dispneia
deve ocorrer com 48 a 72 horas, e, não ocorrendo, devem ser procurados motivos de
falha, tais como escolha inadequada do antibiótico, dose errada ou complicações. Não
há resistência do pneumococo a penicilina, nos casos de pneumonia.
