KEY POINTS
● eliminar as toxinas ou dejetos resultantes do metabolismo corporal: uréia, creatinina, ácido úrico,
etc;
● atuar como órgãos produtores de hormônios: eritropoetina, que participa na formação de glóbulos
vermelhos; a vitamina D, que ajuda a absorver o cálcio para fortalecer os ossos; e a renina, que
intervém na regulação de pressão arterial.
A creatinina sérica é o marcador de diagnóstico mais comumente utilizado para a estimativa da taxa de
filtração glomerular (TFG) na rotina clínica. Devido às interferências e limitações pré-analíticas e analíticas
importantes, a creatinina não pode ser considerada exata e, após estudos, surgiu a cistatina C, que é
produzida em concentrações constantes por todas as células nucleadas e filtrada livremente no glomérulo.
Portanto, a concentração sanguínea de cistatina C depende quase inteiramente da TFG e não é
substancialmente afetada por dieta, estado nutricional, doenças inflamatórias ou malignas e nem pela massa
muscular.

Aspectos positivos:
➢ Fácil de lembrar e requer apenas aritmética básica;
➢ Esta equação considera 4 variáveis: SCr, idade, peso e sexo.

Aspectos negativos
➢ Desenvolvida para estimar a depuração de creatinina, na presunção de que a depuração de creatinina era uma
medida direta da TFG, o que não é (RAMAN et al., 2017);
➢ O que realmente estima é CrCl/24h e não TFG (VEGA et al., 2021)
➢ Foi desenvolvido em 249 pacientes hospitalizados com função renal estável e validado em 236 pacientes (96%
homens). Destes, 59 (23,7%) tinham idade≥70 anos e 17 (6,8%) de > 80 anos (MOLITORIS et al., 2017);
➢ A fórmula incorpora o peso corporal como reflexo da massa muscular superestima a TFG em indivíduos
obesos ou edematosos (GARASTO et al., 2014);
➢ Método utilizado para determinar a SCr no desenvolvimento da fórmula é diferente dos utilizados atualmente
(creatinina padronizada pelo IDMS) (FLAMANT et al., 2012).
MDRD (1999)
Aspectos positivos:
➢ Mede TFGe e não a depuração de creatinina, vantagem do MDRD sobre o CG
➢ Não requer peso corporal
➢ Validado em relação à TFG real medida (calculada usando 125I-iotalomato).
Aspectos negativos:
➢ Desenvolvida e validada internamente em uma população limitada de DRC;
➢ Não incluiu ninguém com idade superior a 70 anos;
➢ Excluiu pacientes com função renal normal, e a proporção de diabéticos (6%) também foi pequena.
Uma das limitações dessas análises é a definição de idoso, pois, por exemplo, pacientes com idade
entre 65 e 70 anos provavelmente apresentam fisiologia significativamente diferente daqueles com
idade superior a 80 anos.
BIS 1 (2012)
➢ Todos os indivíduos tinham idade > 70 anos e a média de idade foi de 78,5 anos
➢ A equação BIS1, como MDRD e CKD-EPI, é baseada em creatinina
(RAMAN et al., 2017)
➢ Desenvolvido e validado em 570 pacientes brancos ≥70 anos (média 78,5) na Alemanha, com DRC
leve ou moderada, com baixa representação de pacientes com TFG < 30 ml/min/ 1,73 m2. Eles
descobriram que TFG obtido com essas fórmulas, quando comparado ao TFG medido com ioexol, teve
um comportamento muito melhor em tendência, precisão e exatidão do que MDRD e CKD-EPI, que
superestimaram a TFG (SCHAEFFNER et al., 2012)

A avaliação da função renal na pediatria é de extrema importância para predizer a dosagem de medicamentos,
garantir a terapia medicamentosa segura e detectar precocemente lesões renais agudas.
★ Particularidades:
➢ Os prematuros apresentam dificuldades adicionais na avaliação da função renal porque a nefrogênese, que
normalmente continuaria até a 36ª semana de gestação no útero, é interrompida no nascimento prematuro
(ABITBOL et al., 2016);
➢ A transição pós-natal mais marcante é representada por um rápido aumento no fluxo sanguíneo renal e um
consequente aumento na taxa de filtração glomerular (TFG) nas primeiras 2 semanas de vida (TUFRO;
GULATI, 2016)
➢ A TFG aproxima-se dos níveis do adulto entre 1 e 2 anos e amadurece mais rapidamente do que as
funções tubulares, resultando em um desequilíbrio glomerulotubular específico para neonatos (TAYMAN et
al., 2011)

➢ A medição da TFG em lactentes jovens é onerosa e altamente impraticável devido às dificuldades de coleta de
urina e sangue e administração de marcadores exógenos nessa população vulnerável;
➢ Apesar de suas limitações bem reconhecidas, a SCr continua sendo o biomarcador renal no qual os
farmacologistas clínicos e os clínicos confiaram principalmente para estimar a TFG na pediatria;
➢ A diretriz KDIGO, de manejo de DRC, sugere a equação de Schwartz para estimar a FG em crianças
(MUHARI-STARK et al., 2018)

Amputação de membros inferiores
➢ Pacientes submetidos a amputação não traumática de membros inferiores são frequentemente polimedicados e
sofrem de redução da função renal;
➢ Apresentam altas taxas de comorbidade de doenças cardiovasculares, diabetes e inflamação, o que pode
acelerar a perda da função renal;
➢ Os medicamentos frequentemente utilizados, incluem: opióides, analgésicos adjuvantes, antitrombóticos e
anti-inflamatórios não esteroides que são eliminados principalmente pelo rim.
Pacientes submetidos à amputação apresentam risco particularmente alto de estimativa incorreta da função
renal com base na creatinina, devido à desnutrição e perda de massa muscular, o que pode levar à prescrição
inadequada de medicamentos de eliminação renal.

O estudo foi conduzido pela Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI), publicado online em 4 de
novembro de 2021 no idioma inglês e com acesso aberto.
Os autores desenvolveram novas equações eTFG sem raça, usando dados de dois conjuntos de dados:

Projetou-se a prevalência de doença renal crônica (DRC) e estágios da TFG em uma amostra de adultos norte-americanos, usando
as equações atuais e novas.
