Pressão Intracraniana
Onda de PIC tem 2 componentes:
• Sistólico e Diastólico
P1 – repercussão da onda sistólica arterial
P2 – adaptação pulsátil do tecido cerebral
P3 – transmissão da onda dícrota
• P2>P1>P3
Em situação saudável – variação da amplitude de pressão de pulso da PIC +/- 2 a 3 mmHg. Varia entre valores
de 10 a 15 mmHg.
Herniação depende
• localização do efeito de massa
• tempo de desenvolvimento do efeito
• Sinais precoces:
• Ausência de alteração pupilar, limitação do olhar para cima,
confusão e alteração do estado de consciência, hemiparésia
contralateral, respiração de Cheyne-Stokes
• Sinais tardios:
• Anisocoria e alteração da reatividade pupilar, alteração do olhar
conjugado. Coma, descorticação até descerebração. Desregulação
térmica, instabilidade hemodinâmica e possível diabetes insípida
