Definitions
•Definição de sepse grave permaneceu a mesma
– Sepse com disfunção orgânica
– Disfunção orgânica definida pelo critério SOFA
•Características do choque séptico
– Pressão arterial média <65 mm Hg
– Pressão sistólica <90 mm Hg
– Queda da pressão sistólica >40 mm Hg do início
– A despeito de adequada expansão volêmica
– Na ausência de outras causas de de hipotensão

Manejo de Sepse Grave e Choque Séptico
Vasopressor Therapy
•Adicionar ou substituir epinefrina por NE
quando um agente adicional é necessário para
manter a BP adequada (2B).
•Vasopressina até 0,03 ug / min pode ser
adicionado a NE; não recomendada como único
vasopressor inicial.
• Supportive Therapies:
– Níveis de glicemia <180 mg / dL (1A)
– Terapia de Substituição Renal e HD equivalente
(2B)
– VAP, DVT / PE, profilaxia de úlcera estresse,
nutrição enteral
– Definir metas de atendimento
